Referring Doctors

Requirements for referral:

– The patient should be active in your practice.

– The referral must be in writing for the requested services and signed by the doctor. This can be faxed, emailed or hand carried by the patient, to their appointment. Our referral pads include a copy for your chart records. If you are in need of our referrals, please contact our office.

X-rays:

A current panorex x-ray, taken within the last year, is preferred. Periapical x-rays are acceptable for single tooth extractions, as long as the related root, bone structure and/or nerve are completely visible.

If sending an original x-ray, please let us know so we can duplicate the x-ray and return the original to your office at the conclusion of treatment.

If current x-rays are not available, please inform our office and one will be taken at the patient’s consultation/examination appointment.

At any time, we will be happy to duplicate, for your records, any x-rays taken by our office, upon request.

Occasionally, our surgeon may refer the patient to have a CBCT. Our office will provide you with a copy along with our findings.

To insure all parties remain informed regarding each patient’s treatment status, a report will be sent for your records.

REFERRING DOCTORS SHOULD FEEL FREE TO CONTACT AND DISCUSS THE TREATMENT WITH THE SURGEON AT ANY TIME!
